PMFT Application Privacy Notice ALT Unlimited collects the information you provide in the PMFT application, including your name, contact information, age, location, program goals, plant-medicine and facilitation experience, prescribed medications, referral information, and questions. This information is used to review program eligibility, support applicant safety, communicate about the application, administer the PMFT application process, and evaluate whether former applicants may be a good fit for future ALT Unlimited programs. Applications are transmitted directly from the PMFT website to ALT Unlimited's Airtable workspace. Application answers, medication information, and consent text are not intentionally sent to website analytics, marketing services, or public logs. Access is limited to authorized program personnel who require the information for application review and administration. Airtable stores the application record, the exact consent wording acknowledged, the consent version, and the acknowledgment timestamp. Retention: All applications, including accepted, declined, and withdrawn applications, are retained indefinitely. ALT Unlimited does not routinely delete application records. Applicants may contact ALT Unlimited about corrections or questions concerning their application data. Email updates are optional and separate from the application consent. If an applicant checks the optional email-updates box, only the applicant's name and email address are synchronized to Mailchimp. Sensitive application answers are never sent to Mailchimp. Applicants can unsubscribe from marketing email at any time. To request corrections to a name, phone number, or email address, or to ask a privacy question, email Contact@AltheaLawtonThompson.com.
